Botox vs. Filler: Which One Is Right for You?

Botox and filler are often confused — but they work completely differently and treat different problems. A Miami dermatologist breaks down which one you actually need.

"Should I get Botox or filler?" is one of the most common questions we hear at Miami Skin Center — especially from first-timers who've done a lot of research and still feel confused. The two products get lumped together in conversations and Instagram posts, but they do completely different things. Here's how to think about them.

What Botox actually does

Botox (and its siblings — Dysport, Xeomin, Daxxify) is a neuromodulator. It temporarily relaxes specific muscles by blocking nerve signals. When those muscles can't contract, the overlying skin stops folding into expression lines.

Treats: Dynamic wrinkles — the kind that appear when you make expressions:

  • Forehead horizontal lines
  • Glabellar lines (the "11s" between the brows)
  • Crow's feet
  • Brow lift (strategic placement)
  • Lip flip, neck bands, underarm sweating (off-label but effective)

Does not treat: Volume loss, hollowing, or skin laxity.

Longevity: 3–4 months for most areas. Daxxify can last 5–6+ months.

Good to know: A balanced three-area treatment is quick and straightforward; your personalized plan is reviewed during your consultation at a board-certified practice.

What filler actually does

Dermal fillers (most commonly hyaluronic acid-based — Juvederm, Restylane, Sculptra, Radiesse) add volume, lift, and structure to areas where fat, bone, and collagen have naturally diminished with age — or where you want enhancement.

Treats:

  • Volume loss in cheeks
  • Under-eye hollowing (tear troughs)
  • Lip enhancement — size and/or shape
  • Jawline definition and chin projection
  • Nasolabial folds (the lines from nose to mouth)
  • Hand rejuvenation

Does not treat: Muscle-driven wrinkles that appear with facial movement.

Longevity:

  • Hyaluronic acid fillers: 6–18 months depending on location and product
  • Sculptra (collagen stimulator): results build over 3 months and last 2+ years
  • Radiesse (calcium hydroxylapatite): 12–18 months

Good to know: Filler is planned by the syringe, and treatment plans vary widely based on your goals — we map a plan together during your visit.

The Miami aesthetic: natural, not frozen

Miami has a reputation for bold aesthetics — and that reputation isn't always flattery. At Miami Skin Center, our philosophy is the opposite: results should make you look rested, refreshed, and like yourself at your best — not like you've had work done.

  • Assessing your face in motion, not just at rest
  • Using Botox conservatively to preserve expression
  • Building filler gradually over treatments rather than over-filling at once
  • Considering the face as a whole unit, not isolated areas

The "pillow face" and frozen forehead you might be worried about are the result of poor technique and over-correction, not the products themselves.

Who needs Botox, who needs filler, who needs both

You likely need Botox if:

  • You have lines in the forehead, between the brows, or around the eyes that are present when you're expressionless (early) or deep at rest (later)
  • You're in your mid-to-late 20s and want prevention
  • You've tried skincare and still notice muscle-driven lines

You likely need filler if:

  • Your cheeks look flatter than they used to
  • You have under-eye hollowing or dark circles with a hollow cause
  • Your lips have lost definition or you want them fuller
  • Your jawline is less defined than before

You likely need both if:

  • You're 35+ and noticing multiple changes
  • You want the most natural, comprehensive rejuvenation

The two products are synergistic. Filler without Botox in a patient with strong muscle activity can actually increase breakdown of filler in certain areas. Botox without filler in a patient with significant volume loss addresses wrinkles but misses the structural issue. Together, they address the face the way it actually ages.

Consultation is everything

The most important thing you can do before any aesthetic treatment is choose the right injector. In Miami, there are many options — ranging from board-certified dermatologists and plastic surgeons to med spas with varying levels of oversight.

We recommend a provider who:

  • Takes photos before every treatment
  • Discusses your goals in your preferred language (English or Spanish — both available at our office)
  • Has a conservative hand and a long-term treatment plan, not a same-day sales pitch
  • Can manage complications if they arise

Bottom line

Botox treats muscle-driven wrinkles. Filler treats volume loss. Most people benefit from a thoughtful combination of both, customized to their face — not a one-size-fits-all package. Book a consultation, come in with your questions, and leave with a plan that fits your goals and your face.
